Design-Build Guaranteed Maximum Price Project Scope:
Renovation and expansion of the Dental Clinic in Building 5300 at Foothill College. Temporary Housing at Building 2200. Path of travel improvements between Parking Lot 5 and the Dental Clinic entry. Accessibility upgrades such as parking space and slope modifications, and nearby restroom improvements.
BREAK OUT THE FOLLOWING ON YOUR BID:
1. Cost for 5300 Project
2. Cost for 2200 Temp Housing Project
3. Your EMR (WC Mod Rates) for the past 3 years (2023-2025)
INCLUDE THE FOLLOWING DOCUMENTS WITH YOUR BID:
• PSA forms (Appendix A & C in the body of the PSA) - filled in & signed by a contract signer
• Proof of CARB Compliance or note on your bid that you do not use diesel equipment
Note: There are no Alternates at this time.
This project is subject to a Project Stabilization Agreement (PSA)
Please submit filled in PSA forms (Appendix A & C in the body of the PSA) with your bid.

New as of 1/1/24: SUBMIT CURRENT CARB CERTIFICATIONS WITH YOUR BID! New California Air Resource Board or C.A.R.B. requirements apply to all subcontractors with equipment on-site. Current certificates of compliance from the Air Resource Board's D.O.O.R.S. website and compliant equipment on-site will be required / monitored at all times by Alten staff.
