We are looking for a DBE to bid for 10% of total. The Los Angeles County Metropolitan Transportation Authority (LACMTA) has established a contracting opportunity to provide the following: Workers' Compensation Utilization and Peer Review service to ensure any and all request for medical
treatment, medications, medical diagnostic testing or surgery from providers are appropriate and follow medical based guidelines as well as all legal and statutory requirements.

Please provide names and addresses of DBE firms; DBE goal, DBE firm must be certified in a NAICS code applicable to the scope of work the firm would perform on the contract; Dollar commitments of each DBE firm participating;
