This is a Union Project.

Description:

Perform all demolition work needed to complete the safety fence installation.

Installation of the safety fence at the Upper Level (UL) and Lower Level (LL).

Provide a mock-up of a typical Upper Level Safety Fence section, LL Safety Fence section, and typical edge treatment at an opening/penetration.

Providing access and assistance to the Authority and its representatives to inspect the existing railing and parapet conditions at the UL and LL eastbound and westbound roadways.

Furnishing and installing all safety fence custom items fabricated from steel and stainless steel sheets, plates, bars and miscellaneous shapes for fencing posts.

Complete survey of all existing utilities and structures which may interfere with the installation of the safety fence.

Perform HAZMAT survey and screening of any suspect ACM and Lead Containing Material.

Maintenance and Protection of Traffic (MPT)

Required Forms: Forms are due by 6/22/20

1) Intent to Perform as Subcontractor/Subconsultant (Form 15A.4)
Fill in form but leave the amount blank

2) Appendix I - Statement of Qualification of Subcontractor/Subconsultant/Supplier/Fabricator
Follow instructions on the form

3) Appendix K - Contractor Responsibility Form
(Who should complete and sign the Contractor Responsibility Form? This form must be completed for all contracts of $250,000 or greater. In addition, contractors and known subcontractors in contracts involving "Special Circumstances" must complete this form. "Special Circumstances" are defined herein as contracts or subcontracts in excess of $100,000 in the following areas:painting; scrap; hazardous materials; concrete; lead; asbestos; inspection and testing; security-related projects; carting; and textiles and garments; or such area
the Authority may designate from time to time. In addition, all known subcontractors having subcontracts in excess of $1 million must complete this form.

4) Staffing Plan
Can use either total work force or only the staff for this project

5) Prove copy of your MBE/WBE certifications by the Empire State Development Corporation (ESDC)

The Bidder will be required to quote in compliance with all project plans and specifications. Non-Disclosure Agreements are provided when applicable. Addenda is posted and noticication is sent to all invited bidders.

Insurance Requirements
Upon award, Subcontractors must provide insurance in accordance with our subcontract agreement. A copy of the subcontractor agreement can be found in SmartBidNet Project Plan Room.

Bonding Requirements
For subcontracts the Bidder shall furnish Performance and Payment Bonds equal to the full subcontract price. For Material Contracts, the bidder shall furnish Supply Bonds in amount equal to full Material Contract price. Bond premiums are reimbursable by Kiewit/Mass. Electric Construction Co.
