Disciplines of Work but not limited to: Aggregates; Clearing and Grubbing; Landscaping; Testing; Asphalt; Precast Concrete Slabs; Saw cutting; Driving Piles; Fabrics; Ready Mix Concrete; Furnish & Install Rebar; Shear Studs; Flatwork; Cleaning of Stone Masonry & Concrete Surfaces, Remove & Reset Stone Masonry; Structural Steel Replacement; Various Bearings; Painting; Barriers; Scuppers; Downspout System (Ductile Iron); RCP; Timber & Lumber; Manholes; Basins Storm water Treatment System; Various Fencing; Pavement Marking; CPM; Surveying; Electrical Subcontractor; Pest Control; Engineer's Field Office; Mounted & Overhead Signage.

Provide References on past/current projects if you have not provided previously along with your EMR (Safety Rating).

Please provide a price proposal to Furnish/Install or Supply your scope or supply of work including but not limited to work & jobsite delivery; all in accordance with & conforming to the project contract documents & add'l requirements necessary to complete the work as specified herein. The referenced work scope shall also be performed to comply with the requirements of the project schedule, milestones, including work staging & phasing, work restrictions & constraints, site access conditions & be coordinated with all other aspects of the project.

A copy of the Subcontract/Purchase Order ("Agreement") your firm will be required to execute if awarded the contract can be found on the below DBox Site. You must base your bid upon the contractual terms & conditions as they are reflected in the Agreement, without amendment. The upstream contractual environment on the Project is demanding & the Joint Venture will not entertain revisions to the contractual boilerplate, which should instead be reflected in your bid price.

All DBE'S participating in this bid, MUST INCLUDE NYSDOT DBE WORK CODES & CERTIFICATION WITH BID. CERTIFYING DIRECTORY THE NYSUCP DBE DIRECTORY https://nysucp.newnycontracts.com ALL DBE'S MUST REFERENCE IN PROPOSAL LINE & ITEM NUMBER ON YOUR PROPOSAL.
